This Child / Minor Booking Waiver & Parental Consent (“Agreement”) is entered into by the undersigned parent or legal guardian (“Guardian”) on behalf of the minor child (“Minor”) receiving services at The Living Spa and Wellness, LLC (“The Living,” “we,” “us,” or “our”).
By signing or electronically accepting this Agreement, the Guardian acknowledges and agrees to the following:
Consent to Treatment
- I certify that I am the parent or legal guardian of the Minor and that I have full legal authority to provide consent for the Minor to receive non-medical spa and wellness services at The Living Spa and Wellness, LLC.
- I hereby give permission for the Minor to receive services including, but not limited to, massage therapy, bodywork, facial treatments, and/or other non-medical wellness services as selected or approved by me.
Non-Medical Services
I understand and acknowledge that all services provided by The Living Spa and Wellness, LLC are non-medical and are not a substitute for medical diagnosis, treatment, or care by a physician or other licensed medical provider.
Health Disclosure + Responsibility
- I certify that I have disclosed all known medical conditions, allergies, injuries, sensitivities, medications, or other relevant health information regarding the Minor that could affect the safety or appropriateness of services.
- I agree to immediately inform the practitioner of any changes in the Minor’s health condition before or during any session.
- I understand that The Living Spa and Wellness, LLC reserves the right to modify, postpone, or decline services if contraindications or safety concerns are present.
Assumption of Risk
I understand that spa and bodywork services involve physical touch and manipulation of soft tissue and may include, but are not limited to, temporary soreness, redness, sensitivity, light bruising, or fatigue.
Release of Liability
To the fullest extent permitted by law, I, on behalf of myself and the Minor, hereby release, waive, and discharge The Living Spa and Wellness, LLC, its owners, members, managers, employees, contractors, and practitioners from any and all claims, demands, damages, or causes of action arising out of or related to the Minor’s participation in services, except in cases of gross negligence or willful misconduct.
Indemnification
I agree to indemnify and hold harmless The Living Spa and Wellness, LLC from any claims, damages, or expenses (including reasonable attorney’s fees) arising from inaccurate or incomplete health information provided by me or from the Minor’s participation in services.
Supervision & Attendance
I understand that The Living Spa and Wellness, LLC may require a parent or legal guardian to be present for the duration of the Minor’s appointment, depending on the service and the Minor’s age, and I agree to comply with all spa policies regarding minors.
Consent to Modify or Decline Services
I acknowledge and agree that services may be adjusted, postponed, or declined at the practitioner’s discretion to uphold client safety, therapeutic integrity, and the highest standard of care.
